*This practice is a Fee For Service entity and considered out of network with all insurance companies. There are no hidden costs and all services are provided in good faith with the utmost value and service.
We do accept HSA/FSA Cards
If you would like to attempt to get reimbursement from your insurance company, we can provide a Superbill for you to submit to your insurance provider after payment for service. It is suggested that you call your insurance company to better understand what they will reimburse for out of network physical therapy services
